What is a myomectomy?
A myomectomy is a surgical procedure that removes uterine fibroids - also called myomas or leiomyomas - while leaving the uterus in place. It is the procedure of choice for patients who would like to preserve the possibility of pregnancy or who prefer to avoid a hysterectomy.
For background on the condition itself - causes, symptoms, and non-surgical options - see Uterine Fibroids Treatment in Lisbon.
Types of myomectomy
The right surgical approach depends on the number, size, and location of the fibroids, as well as your overall health and reproductive goals.
Hysteroscopic myomectomy
Performed through the cervix, with no abdominal incisions. It is the preferred approach for submucosal fibroids that distort the uterine cavity and is often a same-day procedure with rapid recovery.
Laparoscopic myomectomy
Performed through small abdominal incisions using a camera and specialised instruments. Compared with open surgery, it usually means less post-operative pain, smaller scars, and a shorter hospital stay. It is suitable for many intramural and subserosal fibroids.
Open (abdominal) myomectomy
In selected cases - very large fibroids, very numerous fibroids, or specific anatomical situations - a traditional abdominal incision may be the safest option. The decision is always discussed in detail before scheduling surgery.
How a myomectomy is planned
- Detailed consultation to understand symptoms, fertility goals, and previous medical history
- Pelvic ultrasound to map the fibroids
- Saline infusion sonography or pelvic MRI when more detail is required
- Blood tests to assess anaemia and iron stores before surgery
- A personalised plan with the recommended approach, expected recovery, and follow-up
Recovery after myomectomy
Recovery is shaped by the surgical approach:
- Hysteroscopic myomectomy - most patients return to daily activities within a few days.
- Laparoscopic myomectomy - light activity within 1 to 2 weeks, full activity around 2 to 4 weeks.
- Open myomectomy - a longer recovery, typically several weeks, with gradual return to activity.
Your surgeon will give you tailored guidance on activity, work, exercise, and any waiting interval before trying to conceive.
Myomectomy and fertility
Because myomectomy preserves the uterus, it is often chosen by patients who plan to become pregnant. When fibroids are part of a broader fertility picture, Dra. Joana Faria coordinates care with reproductive medicine teams so that the surgical plan supports your overall path.
